Forms

These forms are necessary for Camp.

The 2 main forms are:

“First Aid and Medication Form” page 1 and the “Release and Waiver Form” page 2.

"Camper Pickup Form" for Overnight Camp or for Day Camp depending on your camper.

The forms are in PDF format and fill-able, which means you can fill it out on a laptop or tablet device (not a cell-phone) and email it back to campredarrowinfo@gmail.com, or print it and bring the filled-out Forms with you to Registration at 8:30 am on the day of check in for camp.  Bringing the completed forms with you will help the registration line go smoothly. If you do not fill out these forms (or other forms) until registration, you will be asked to leave the registration line until all the forms are filled out. This can easily add another hour to your time on that day.
